DEMAND FOR A JURY TRIAL

Any information available how to apply for a jury trial at the county court will be much appreciated.
I will proceed with this action anyway and I will post updates. Thanks!

Just in case you don't know . . . although I hope you do.
The Criminal Justice Act, 1820 or 1840 ( I forget exactly) says that all officials must do the job they're employed to do and the Interpretations Act, 1978, section 3 says that every Act must be given Judicial notice and each official must fulfil his functions when called upon to do so. This is when may means must - see Julius V the Bishop of Norwich. There are others.
The Bill of Rights, 1689, says that the pretended power of suspending laws is unlawful.

My hearing is postponed for one week,
I want to ask the court to have a jury trial. Do I have to just fill a general application form including my statement /demand notice ?
Do I need to pay a fee?
Has anyone sent a jury trial demand to a county court?
